Immunization Requirements
Before Enter Preschool or Toddler Center
The State of California Department of Health Services
requires that all children are properly immunized before entering preschool.
Below is a table to follow of the required immunizations your child needs to complete in order to enter preschool.
18 months through 4 years old:

| Measles, Mumps and Rubella |

| HIB (Meningitis) |
4 Doses |
| Varicella |
1 Dose |
| Hepatitis B |

| TB (Tuberculosis - Mantaux Test) |
1 Test with Result |
TB Test is acceptable if given within 12 month period, prior to enrollment.
All immunizations have to be properly dated with the month, day and year.
If your child is not properly immunized, she/she will not be allowed to enter school.
