1. All prescription medications must be in the pharmacy container, properly labeled with the name of the child, doctor's name, name of the pharmacy, name of medication, dosage and time medication is to be given. Medication that is listed on the bottle 'three time a day' will be given every eight hours.

2. Do not send sample bottles of medicine.

3. Parents must bring medication into the nurse's office, not the children

4. A medical authorization slip must be filled out and signed by a parent in order for medication to be dispensed.

State law prohibits nurses from making diagnoses or prescribing treatment. However, please feel free to ask about identification of health conditions, nutrition information, or physical activity recommendations for your child.
