Middle School Note:
The California School Immunization Law requires students entering 7th grade to have three hepatitis-B shots and a second measles (or MMR shot). A Td booster is also recommended. Children who have not started their hepatitis B shots before the first day of 7th grade will be excluded from school.
Also, no student may have medication in his/her possession or carry it in a backpack at any time. This is not only the law, it is for the protection of all students. Please bring any medications to the office in the original container (including over the counter medicine). Prescription medications, including inhalers, must be properly labeled with the child's name, name of medicine, doctor's name and pharmacy information. Parents MUSR fill out a Request to Administer Medication form before medication can be given.
